• Co-payments and outstanding balances are due at the time of service.
• For uninsured patients, payment is required in full at the time of visit.

• If you access our patient portal, additional terms apply to account creation, secure messaging, and telehealth services.

7. Prescription Policy

• Advance Requests: To ensure timely processing, prescription refill requests should be made well in advance- ideally at least one week prior to running out of medication.
• Controlled Substances: Prescriptions for controlled substances are not guaranteed and are subject to strict federal and state regulations. Additional conditions may apply.
• Pharmacy Coordination: Please confirm with your pharmacy before requesting a refill to avoid duplicate requests.

8. Intellectual Property

• All content, including text, graphics, and logos, is the property of Capital Care Clinic unless otherwise stated.
• Unauthorized use, reproduction, or distribution is prohibited.
